Policy number and title:

COMMISSIONER'S DIRECTIVE 041 - INCIDENT INVESTIGATIONS

Why is this policy changing?

CD 041 on Incident Investigations will be amended because of a procedural change to the review by the Senior Deputy Commissioner and members of the CSC's Executive Committee of national investigations as well as other relevant information from fact finding reports.

The technical change results from a recommendation of the Correctional Investigator's Investigation Report into the death of an inmate in a medium-security institution.

What is changing?

Paragraph 50 will be amended to: "The Senior Deputy Commissioner and members of the CSC's Executive Committee will review findings, including information from relevant fact finding reports related to the incident that was investigated so that they are fully aware of key issues from different investigations/reviews, and approve recommendations and action plans developed in response to national investigation reports and formally close these investigations."

Accountabilities?

The accountabilities relate to the Director General of Incident Investigations.

Who will be affected by the policy?

All staff in the Incident Investigations Branch.

Expected cost?

This change will not affect the overall costs of the investigative process beyond the present resources dedicated to this function.

None

Implementation:

This change to the policy will come into effect immediately. CD 041 will be updated in the near future.
