COMMISSIONER'S DIRECTIVE 041 - INCIDENT INVESTIGATIONS
CD 041 on Incident Investigations will be amended to reflect procedural changes with regard to investigations convened for Deaths by Natural Cause.
These changes stem from an EXCOM decision in 2005 that an alternative investigative process would be appropriate for cases of Deaths by Natural Cause.
The Health Services Sector will now assume responsibility for a Mortality Review Process of Deaths by Natural Cause of inmates residing within CSC institutions or residing with residency conditions in the community using the procedures outlined in the CSC Guidelines, Mortality Review Process, which will be available on the Health Services InfoNet site in the near future.
The changes to CD 041 were developed in consultation with the Health Services Sector, National Headquarters.
A definition of the Mortality Review Committee will be included in the Definition Section of CD 041.
Paragraphs 24, 25, 31, 34, 37, 45, 49, 50, and 53 will be amended, as well as Annex A, to include references to the Mortality Review Process where applicable.
The accountabilities relate to the Senior Deputy Commissioner and the Director General, Incident Investigations, in deciding which incidents will be investigated; to the Director General, Clinical Services, in overseeing the reviews; and to the Assistant Commissioner, Health Services Sector, in approving action plans, presenting the results of these reviews to EXCOM, and closing the reviews.
Chiefs of Health Services in the institutions, Institutional Heads, Managers of Clinical Services in the regions, staff in the Incident Investigations Branch, and in the Health Services Sector.
These changes will not affect the overall costs of the investigative process beyond the present resources dedicated to this function.

These changes to the policy come into effect immediately. CD 041 will be updated accordingly.
