DVD/Video User Agreement
"No More Victims, No one is disposable, Forging Community"
Below, the videos/DVD, "No More Victims, No one is disposable, Forging Community" are referred to as COSA DVD/Video's. COSA refers to Circles of Support and Accountability.
In completing this form, the undersigned agrees to the following stipulations:
- The COSA DVD/Video's are videos created for the purpose of educating the community regarding the Circles of Support & Accountability model for assisting high-risk sexual offenders in their re-entry to society following incarceration. The principal target audience is community volunteers; however, members of the professional community (e.g., psychologists, physicians, social workers, law enforcement) may also find the COSA DVD/Video's instructive.
- The COSA DVD/Video's remain the sole copywritten property of the Correctional Service of Canada, Chaplaincy Division.
- The COSA DVD/Video's will not be reproduced in any form without the explicit written permission of the copyright holder.
- The COSA DVD/Video's will not be broadcast in any form without the explicit written permission of the copyright holder.
- The COSA DVD/Video's will be screened for educational purposes only, unless the copyright holder has approved other use in writing. No screening
or admission fees may be charged.
- Any breach of the above stipulations constitutes an abrogation of this agreement, and will result in forfeiture of the use of The COSA DVD/Video's and could result in legal action.
