Intake Assessment Process and Correctional Plan Framework

AUTHORITIES

PURPOSE

  • To establish the intake assessment and Correctional Plan framework for:
    • all offenders sentenced to two years or more
    • provincial/territorial offenders transferred to federal jurisdiction under Exchange of Services Agreements and
    • young persons transferred to federal jurisdiction pursuant to sections 76, 89, 92 or 93 of the Youth Criminal Justice Act
  • To outline the process for determining an offender’s security level and penitentiary placement

APPLICATION

Applies to staff responsible for the intake assessment, the development of the Correctional Plan and the penitentiary placement of offenders

RESPONSIBILITIES

  1. The Regional Deputy Commissioner will ensure:
    1. procedures and agreements are in place for provincial authorities to inform CSC of new federal sentences and long-term supervision orders
    2. intake assessment and correctional planning policies are effectively communicated to operational units and monitored
    3. operational reviews of policies and procedures are conducted on a regular basis
    4. policy gaps are promptly communicated to the Assistant Commissioner, Correctional Operations and Programs
    5. significant staff safety issues are dealt with pursuant to the Employee Protection Program Guidelines.
  2. The Assistant Deputy Commissioner, Institutional Operations, will:
    1. provide support and advice to the Assistant Wardens, Interventions, responsible for the intake assessment process, correctional planning of the sentence and penitentiary placement
    2. provide support and advice to the Assistant Wardens, Operations, responsible for the admission and immediate needs process.
  3. The Institutional Head responsible for the reception of offenders and the intake assessment process:
    1. may refuse to admit a person into the penitentiary who does not have a medical certificate
    2. will ensure the person delivered to the institution is the person referred to in the warrant of committal and that this is verified
    3. will ensure that health services are available when required, nursing assessments are completed within 24 hours of initial arrival (including following a court return, an inter or intra-regional transfer and a warrant of suspension), and mental health screenings are carried out pursuant to CD 800 – Health Services, CD 843 – Management of Inmate Self-Injurious and Suicidal Behaviour, CD 850 – Mental Health Services and the Institutional Mental Health Initiative (IMHI)
    4. will ensure processes outlined in CD 784 – Information Sharing Between Victims and the Correctional Service of Canada are respected
    5. will ensure a process is in place for the development, maintenance and monitoring of the Correctional Plan
    6. will ensure procedures for high profile offenders are followed pursuant to CD 701 – Information Sharing.
  4. The Assistant Warden, Interventions, responsible for the intake assessment process and the correctional planning of the sentence will:
    1. ensure appropriate sentence management procedures are in place to verify admissions, calculate eligibility dates, and address issues pertaining to the administration of the sentence pursuant to CD 703 – Sentence Management
    2. establish a process to monitor compliance and effective implementation of policies
    3. implement procedures in support of the intake assessment and development of the Correctional Plan in consultation with the offender, as soon as practicable after his/her admission to a penitentiary
    4. ensure the provision of programs, services, and activities which respect gender, ethnic, cultural and linguistic differences, and are responsive to the special needs of women offenders, Aboriginal offenders, offenders requiring mental health care and other groups pursuant to CD 726 – Correctional Programs
    5. ensure the provision of pre-treatment and awareness programs such as suicide awareness and prevention
    6. ensure that offenders serving sentences of four years or less are provided with programs during the intake process
    7. ensure a process is in place to review re-admissions and requests for re-admissions, and determine the level of reassessment in relation to a change in the offender’s offence cycle
    8. chair the Correctional Intervention Board.
  5. The Assistant Warden, Operations will ensure admission and immediate needs identification assessment procedures are in place, prior to cell assignment pursuant to CD 550 – Inmate Accommodations.
  6. The Manager, Assessment and Interventions/Manager, Intensive Intervention Strategy, will:
    1. participate in the Correctional Intervention Board
    2. ensure institutional case management practices, interventions, and release strategies are in line with requirements and developed in cooperation with Parole Officer Supervisors
    3. provide supervision of and guidance to Parole Officers to ensure the quality of intake reports and to ascertain that Correctional Plans are developed in compliance with this policy
    4. conduct ongoing and regular case conferences and placement reviews to identify and confirm the most effective intake assessment strategies and interventions
    5. ensure any issues related to safety and security are identified and actioned
    6. ensure consistent information sharing practices pursuant to CD 701 – Information Sharing.
  7. The Correctional Manager will:
    1. establish and maintain a quality control process for all documents completed by the Correctional Officer II/Primary Worker
    2. follow up on any outstanding actions identified in the Structured Casework Record
    3. ensure any issues related to safety and security are identified and actioned.

  1. The intake assessment process and Correctional Plan framework is comprised of the following stages:
    1. Preliminary Assessments and Post-Sentence Community Assessments (CD 705-1) – identifies the activities and requirements after federal sentencing
    2. Information Collection (CD 705-2) – identifies the activities and requirements for official information collection
    3. Immediate Needs Identification and Admission Interviews (CD 705-3) – identifies the activities and requirements for addressing the offender’s needs and admission requirements
    4. Orientation (CD 705-4) – identifies the activities and requirements for the offender orientation process
    5. Supplementary Intake Assessments (CD 705 5) – identifies the activities and requirements for supplementary intake assessments
    6. Correctional Planning and Criminal Profile (CD 705 6) – identifies the activities and requirements for the intake assessment reports and the Correctional Plan
    7. Security Classification and Penitentiary Placement (CD 705-7) – identifies activities and processes for determining an offender’s security level and penitentiary placement
    8. Assessing Serious Harm (CD 705-8) – identifies the activities and processes for assessing if the index offence has caused serious harm.

Correctional Intervention Board: a multidisciplinary team that approves interventions and contributes to recommendations or decisions for complex cases. The Interdisciplinary Mental Health Team is considered a component of the Correctional Intervention Board.
